When talking about Candida Arthritis there are a couple of things to look at, septic arthritis caused by disseminated or invasive candidiasis and the link between Candida and Rheumatoid Arthritis. This article is focused in the former of these two conditions. Candida is a pathological fungus that most people associate with yeast infections, in particular the vaginal yeast infection. However it can cause other health problems and in people with weak immune systems and people in critical care in hospital it has a much more serious impact on a person’s health.

What is arthritis?

Arthritis is when the cartilage the lines the ends of our bones has been damaged leading the bones to rub against each other. This leads to the bones being damaged and symptoms such as swelling and pain. There are several things that can cause arthritis including a systemic candida infection.

Candida Arthritis caused by invasive candida

Candida arthritis is not at all common, just because you have arthritis and candida does not mean they are linked, there are only specific circumstances in the case of septic arthritis caused by candida. There are two ways it can occur, a patient in hospital may develop candidemia but go untreated and the candida arthritis develops later, or in some cases despite the candidemia being recognized and treated appropriately, candida arthritis has still occurred. It actually affects children more than adults, about a third of babies that have neonatal candidiasis also have bone or joint problems too i.e candida arthritis.

Who is at risk

Candida arthritis risk groups include the following;

Babies are the highest at risk of candida arthritis or/and candida osteomyelitis. In one review it concluded that at least 85% of the cases were in babies 6 months or younger

Patients receiving organ transplants, having abdominal surgery or those with hematological disorders

Urinary tract manipulation patients

Patients that have had intra-articular injections

Drug users that take brown heroin intravenously

In most cases Candida albicans are responsible but there are cases of candida tropicalis, candida krusei, candida glabrata, candida zeylanoides and candida parapsilosis.

Symptoms of Candida Arthritis

Pain in the joints

Tenderness

In babies the affected limbs may have abnormal positioning

Warmth

Swelling

Usually the areas affected are the knees, but after it can also affect the hips, ankles and shoulders. In adults there is often a delay in presentation of candida arthritis after having candidemia, but in babies it happens together.

Treating candida arthritis

Systemic treatment and topical treatment is usually called for when treating candida arthritis. Anti-fungal treatment to treat the candida such as amphotericin B, or one of the azoles such as ketoconazole or fluconazole. Drug choice depends on the species of candida and the patient’s medical history. If there is an infected prosthetic joint involved this usually has to be removed and replaced with a new prosthesis. Local treatment involves synovial aspiration and if these options fail a synovectomy may be considered which is a surgical operation that removes part or all of the synovial membrane of the affected joint.

Candida can overgrow and cause an infection anywhere on the body including the mouth and throat. Called Thrush when it just affects the mouth, esophageal when it is just the throat or oropharyngeal candidiasis when it is both mouth and throat it is most common in those with HIV or AIDS, patients with already weakened and compromised immune systems. It is quite possible to have an oral yeast infection without it spreading to the throat or esophagus, but this article looks at can candida be present in the esophagus but not the mouth?

Symptoms of each type

Thrush or oral candidiasis has white patches on the tongue, cheeks, sometimes the roof of the mouth, beneath which can be lesions that bleed if touched. There is pain and burning. Esophageal candidiasis is where the infection is just in the throat so yes is the answer to can candida be present in the esophagus but not the mouth, since it has its own term and symptoms. They include difficulty swallowing, feeling like food is caught in the throat, pain, vomiting, nausea, fever and sometimes weight loss. In severe cases it can even cause respiratory problems. Oropharyngeal when both exist has potentially all of those symptoms at once.

How can candida be present in the esophagus but not the mouth?

As said candida can become a problem anywhere, however while it is possible, it is rare for a person to get just a throat yeast infection. In most cases a patient who has candida overgrowth in the esophagus will also have some amount in their mouths.

Treatment

If you are not pregnant azole oral therapy is usually effective, Fluconazole, clotrimazole, posaconazole, itraconazole for example. Other oral treatments include Nystatin oral suspension. For the throat treatment depends on whether the patient is able to swallow, as there are anti-fungal pills that can be given, if they cannot then they are put on IV therapy. Drugs can be the azoles but may also be micafungin, anidulafungin, caspofungin or amphotericin B. If they have a resistance to treatment, the type of candida needs to be checked. Drug choices by the doctor depend on the patient’s medical history, their current health, what other medications they are taking and chances of relapse. Maintenance treatment can be used for some patients.

People with candida overgrowth can present with a wide range of symptoms that may be ignored or thought to be a symptom of something else at first. Candida in fact is being linked to many problems one of those being asthma. This article looks at how candida and coughing occur as a result of asthma being caused or aggravated by Candida overgrowth and what other symptoms you need to be aware of. It also looks at respiratory tract candidiasis as a source of candida and coughing.

Symptoms of the intestines

Often people with candida and coughing will also have intestinal symptoms from an internal overgrowth. Symptoms include having frequent stomach upset, struggling with constipation or diarrhea, and nausea or vomiting. This can be caused by too many toxins in the stomach the candida releases as waste. There can also be blocking and the lower esophagus can become inflamed.

Respiration problems

Obviously asthma causes respiration problems, but having candida and coughing is also indicative of respiration problems. Sometimes the cough starts out as occasional but can soon develop into something far more persistent and difficult to live with. You will find that your cough does not respond to cough medicine and your symptoms worsen at night or when you are reclined. Other respiration symptoms that may come along with candida and coughing are wheezing, difficulty catching your breath, and a feeling of tightness around the chest area as is something heavy is sitting on it, bronchitis, congestion, clogged sinuses, nasal itching, sinus infection, excess mucous, and sneezing.

Why it may be mistaken for the flu

Very commonly a person with candida and coughing will also have some flu like symptoms which is why it may be mistaken for the flu or some such illness at first. You can feel weak, fatigued, have difficulty doing your every day chores or work, have a sore throat and so on.

Other physical signs

Other physical symptoms include;

Oral candidiasis

diaper rash if in an infant

vaginal or penis yeast infection

PMS like symptoms

blurred vision

if the infection is also in the kidneys there may be bloody urine

Respiratory tract candidiasis

There are actually several types of candidiasis that can cause coughing symptoms apart from the aggravation of asthma. These include;

Candida tracheobronchitis – Not a common form and usually only found in patients with weakened immune systems such as those with HIV. Symptoms of this invasive candidiasis are a productive cough, fever and shortness of breath.

Laryngeal candidiasis – Again not common and usually found in patients with oncologic or hematologic malignancies. Symptoms include a sore throat, coughing and being hoarse.

Candida pneumonia – A person is very unlikely to get this on its own, they are more likely to have other systemic candida problems alongside it. There can be lung abscesses, a lot of candida in the respiratory tract, coughing, shortness of breath, fever, breathing sounds.

Treating candida and coughing

If the coughing is from a respiratory tract infection then systemic anti-fungal therapy will be started. This may include fluconazole, caspofungin, anidulafungin, micafungin, voriconazole or amphotericin B depending on how invasive it is and what candida species is causing the infection.

A yeast infection is also commonly referred to as Candidiasis or Candida, or Candida overgrowth. A few years ago it was once known as Monilia or Moniliasis but its name was changed, however some older texts or sources may use that term. Candidosis or oidiomycosis may also be terms you come across. A yeast infection can sometimes be referred to by the causing fungus species, most commonly Candida albicans, but there are other pathogenic candida species that can cause infection in humans, such as Candida glabrata, Candida tropicalis and so on.

There are also different forms of yeast infections and each form has its own name or several names! A mouth yeast infection can also be called oral candidiasis, an oral yeast infection, Thrush, in older texts, oral moniliasis. A vaginal yeast infection may also be called vaginal candidiasis or candidal vulvovaginitis. Wikipedia lists the following names of subcategories for candida;

Sepsis is a term used for a blood infection. Blood infections are very serious medical conditions and can be caused by a number of organisms. Bacterial infections are the most common cause of sepsis, conditions like urinary tract infections, pneumonia, meningitis, appendicitis and cellulitis are all conditions that can lead to sepsis if not treated properly. But viral and fungal infections can also enter the blood stream and cause a blood infection including the fungus candida, the most common fungal blood infection, which is known as Candida sepsis, or disseminated candidiasis, or systemic candidiasis. Candida can enter the blood in a number of ways, but Candida sepsis tends to be a problem patients already in hospital for other conditions experience. It is very hard to treat and has a high fatality rate, possibly as high as 70%.

Weakened immune systems

Those most at risk are those with weakened immune systems such as though with HIV or AIDS, undergoing chemotherapy, diabetics, low birth weight newborns, surgical patients, critical patients, burns victims, transplant patients, drug abusers and places that use needles and do not clean them correctly like tattoo places, ear piercing places are potential sources though less common for fungal and more common for bacterial. According to a report from 1985 common sites for the Candida sepsis to occur from include the urinary tract, and catheters.

Symptoms of Candida sepsis

septic shock

fever from unknown source that does not respond to antibiotics

organ failure

death

Hospital acquired infections

Candida sepsis has become one of the most common forms of hospital acquired infections in recent years perhaps due to a combination of hospitals being more aware of it, and due to new procedures, drugs and equipment that make it possible. In a report that looked at incidence of Candida sepsis 58 patient’s case files were looked at. Their age ranged from 5 days old to 72 years old, showing Candida sepsis has no specific age group. 35 were men and 23 were women. Symptoms of Candida sepsis began to show somewhere between the 13th and 48th hospital day. One third were major burn victims, almost another third again had intra-abdominal infections. There were some surgical patients, and a few cancer patients.

All presented with a fever that did not respond to antibiotic treatment ranging from 101 to 105F. White blood counts were raised in all but four patients. All had intravenous catheters as part of their treatment and this was the cause of the Candida sepsis for some of the cases. 14 patients were not treated correctly and it was only realized when they died that the problem had been Candida sepsis. 23 received the anti-fungal drug nystatin, 9 were given amphotericin B, 7 were given a combination of the two. Only 38 of the patients had their antibacterial treatment stopped. Those that did not died.

The conclusion is that those known to be at high risk of candida sepsis should be administered a preventative drug. Also every effort should be made to maintain sterility when changing dressing, and when inserting catheters. Topical anti-fungal creams should be applied to the skin puncture wound, and fresh sterile tubing and connectors should be used as replacements each day.

This means the fungus has entered the bloodstream and can then spread to around the body, reaching vital organs. Deep candida is something that most people do not have to worry about at all, it is usually hospitalized patients who have weakened immune systems that are affected, such as cancer patients undergoing chemotherapy, organ transplant patients, low birth new born babies or HIV / AIDS patients. The candida gets into the bloodstream via skin catheters, ventilation tubing, tracheotomy sites, or surgical wounds. In those less common otherwise healthy people that get deep candida it can happen of you are a drug abuser, or have been badly burned or wounded.

Symptoms of deep candida can be extremely wide ranging as it depends on what organs become infected. Any of them can be, the heart, liver, lungs, spleen, kidneys, eyes and the brain. Often there is unexplained high fever, patient may exhibits signs of shock or organ failure.
respiratory distress

systemic rash or skin peeling

chills

Deep candida symptoms are very varied but what is important to remember is it is a very serious problem, that can be life threatening.

Candida species

It is estimated that 38% of patients with deep candida die because of it. Though most cases that present with symptoms of deep candida are caused by Candida albicans there are other Candida yeasts that are also a problem including Candida torulopsis, Candida krusei and Candida parapsilosis. This is an important thing to consider for the doctors trying to diagnose symptoms of deep candida because different candida species do not all respond the same way to the same anti-fungal treatments. Different species have different susceptibilities.

Treating symptoms of deep candida

First of all the candida needs to be treated with an anti-fungal drug such as amphotericin B, fluconazole or itraconazole. Surgery may be needed and depending on what organs have been affected depends on what other treatments are used. The key to successful treatment of deep candida is a quick diagnosis. If it is treated before major organs are affected prognosis is a lot better.

Candida krusei is one of the less common though emerging more frequently recently, cause of candidiasis in patients with hematological malignancies and the immunocompromised. It is estimated at being the cause of between 1-3% of cases. Other pathogenic species of candida include candida albicans, candida glabrata, candida dubliniensis, candida tropicalis, candida parapsilosis. There are many species of Candida but most are not pathogenic. Interestingly Candida krusei also has a role in chocolate production!

Treating Candida krusei

Candida krusei is can be resistant to fluconazole, one of the go to medications for systemic candidiasis. Patients with Candida krusei have a high mortality rate though it can be successfully treated with amphotericin B, voriconazole, anidulafungin, caspofungin and micafungin.

Candida krusei in the production of chocolate

When cacao beans have been harvested they then need to be fermented to get rid of the butter taste they have and to break them down. Two fungi are involved in this process, geotrichum and candida krusei. Often these fungi are already in the beans in the seeds or seed pods, but in these modern times, certain strains are used, and each chocolate company has their own selection for flavor and aroma.

The yeast reproduces quickly creating thousands of cells in a small area that then make enzymes which break down the pulp outside the cacao beans. This creates acetic acid which destroys the seed embryo, removes the bitterness and creates a chocolaty smell.

Candida krusei symptoms

Candida krusei is more likely to cause a systemic infection in hospitalized patients rather than in an otherwise healthy person. Symptoms in the latter include a white fuzzy film, burning irritation and itching. It can affect anywhere on the body including the mouth, skin, scalp, ears and genitals.

In weakened patients Candida krusei can lead to gastrointestinal candidiasis, it can invade the respiratory tract, and it can be systemic. Symptoms of systemic candida krusei include fever that is long lasting, skin lesions, infection of internal organs, septic shock. Clinical manifestations include fungaemia, endocarditis, arthritis and endophthalmitis. Candida krusei infections are more common amongst patients who have been administered fluconazole as a prophylactic, leading some to question whether this practice should be continued.

Candida krusei compared to other candida species

In clinical studies Candida krusei is less virulent than candida albicans in terms of forming biofilms on prosthetic and epithelial surfaces. It is also different in its structure, metabolic features and behavior patterns. Due to these differences there are some that say it should be reassigned.

Anidulafungin marketed as the brand name Eraxis from Pfizer was given FDA approval as a treatment for candidemia. Candidemia is a potentially fatal bloodstream infection with a mortality rate as high as 40%, making it the most deadly of hospital acquired blood stream infections.

One in five thousand people are affected by candidemia in the United States alone, leading to about 60,000 cases each year, and these figures are on the rise. Dr Joseph Feczko, the chief medical officer from Pfizer says,

“Bloodstream infections such as candidemia can spread quickly and are very dangerous, especially for patients with weakened immune systems…Physicians treating these seriously ill patients now have an important new treatment in Eraxis.”

Those patients who are more at risk of candidemia and systemic candidiasis (where the yeast infection spreads throughout the body affecting various internal organs) are those with compromised immune systems. Cancer patients receiving chemotherapy, stem cell and organ transplant patients, patients with catheters, those in intensive care, critically ill patients, those on prolonged antibiotic therapy, surgical patients, and HIV / AIDS patients. Patients who acquire candidemia spend up to an extra 10 days in hospital on average, costing an extra $39,000 per patient.

Dr. Annette Reboli, head of the Division of Infectious Diseases at Cooper University Hospital in Camden, New Jersey, and lead clinical investigator says,

“In the clinical trial setting, patients taking Eraxis for the treatment of candidemia had improved efficacy versus those taking fluconazole, making Eraxis an important addition to the options in anti-fungal treatment. In addition, Eraxis has been shown to have a safety profile comparable to fluconazole and to be compatible with many medicines commonly used by patients with candidemia who have other serious health complications.”

Eraxis is part of the class of drugs known as the echinocandins and has also been FDA approved to treat candida peritonitis and esophageal candidiasis. Pfizer has several anti-fungal treatments to its name, Diflucan (fluconazole) being one of the top drugs used against candida infections, and also Vfend (voriconazole) also from the azole type of anti-fungal treatments.

Leaky gut syndrome is believed to be a problem that can happen when a person has candida overgrowth in the digestive system. It refers to the gut wall or intestinal lining becoming damaged which leads to partially digested foods, bacteria and toxins passing through and entering the bloodstream. Microvilli cells are damaged and cannot do their job well enough, which is to make the enzymes that are essential to good digestion. So we are no longer digesting our food properly, which means the body is not getting the nutrients it needs and harmful substances are in our blood. These are seen as invaders by the body and subsequent symptoms can be very unpleasant.

Some believe Leaky Gut Syndrome may have a causing role in conditions such as,

arthritis

chronic fatigue syndrome

Crohn’s disease

diabetes

pancreatic dysfunction

fibromyalgia

IBS

inflammatory bowel

lupus

This is because as the immune system becomes overburdened with the toxins being excreted by the fungus and that are entering the bloodstream the liver can not deal with it and they get flushed back into the blood where they reach other organs, muscle and tissue.

Causes of Leaky Gut Syndrome

Candida overgrowth is not the only cause of Leaky Gut Syndrome. And the problem with it is that some of the causes are also symptoms, and some of the symptoms can become causes. It becomes a nasty cycle that is hard to interrupt. Other causes include,

Diets too low in fiber

Food allergies

Some drugs including antibiotics

Diets that are too high in refined sugar

Poor digestion

Alcohol

Candidiasis

Parasitic infections

Pathogenic bacteria

Deficiency in zinc

Nonsteroidal anti-inflammatory drugs such as aspirin and ibuprofen

Inflammatory bowel disease

Nonpathogenic bacteria, such as Klebsiella or Pseudomonas

Leaky Gut Syndrome symptoms

Headaches

Fatigue

Constipation

Nutritional deficiencies

Candidiasis which causes its own symptoms such as bloating, carb craving, fatigue, allergies, yeast infections, depression, anxiety and more.

Allergies

Skin rashes

Weakened immune system

Moodiness

Flatulence

Bloating

Poor memory

Lack of focus or concentration

Our internal balance

In our intestinal tracts are billions of micro-organisms, and in a normal healthy person these are in balance. A small number are candida and these are controlled by the good bacteria. The good bacteria have a variety of functions, they help the immune system, help keep the gut healthy, help remove toxins from the bowel. The two key important good bacteria are lactobacillus acidophilus and bifidus. The former live mostly in the upper small intestine and the later live more in the large intestine and lower small intestine. These friendly bacteria basically make it harder for unfriendly flora to move in by taking up all the room in there! They help with vitamin production, making short chain fatty acids, detoxifying and enzyme production.

Treating Leaky Gut Syndrome
The important thing is to deal with the cause not just try to patch up the symptoms. There are several options depending on your cause and your preference but the key things to do are get rid of the overgrowth of candida, use digestive enzyme to help get your digestive process running properly again, use probiotics to keep the good bacteria topped up to optimum numbers, eat well and take supplements to replace the nutrients you are lacking.

Candida – Causes Leaky Gut Syndrome when the roots of the pathogenic fungus go through the intestinal tract searching for food. There are several natural treatments that are effective for candida such as acidophilus supplements, caprylic acid, pau d’arco, fresh garlic, and colloidal silver.

Diet – Eat high fiber, whole grains, legumes, leafy greens, uncooked vegetables. Cut out foods candida thrives on such as sugar, refined foods, fermented products and fungus.

Supplements – Essential oils such as borage, evening primrose, fish oils, flax seed oil. Olive leaf extract, grapefruit seed extract, oil or oregano.

Detox – Use detoxifying products to help clean yourself out, psyllium seed and husk can help as can citrus pectin.

Digestive enzymes – These can hep with the symptoms while you are dealing with the Leaky Gut Syndrome. There are several to choose from on the market, and can help with absorbing nutrients and stop partially digested proteins from being absorbed which overworks the liver. Foods can also provide enzymes though it is best to eat them raw, as the cooking can damage them. Foods like mangoes, pineapples, bananas, sprouts, avocados and papaya are all good.

Probiotics – When supplementing with probiotics look for the 2 important ones Lactobacillus acidophilus and bifidus. Taking probiotics to keep high levels of good bacteria will help with digestive problems, stop a candida overgrowth, and help absorb nutrients.

Monilia or Moniliasis is a less used and older term for a Candida infection. It means the same thing as Candidiasis or yeast infection. Candida Albicans Monilia are the names of the group of yeast that cause a yeast infection when there is an overgrowth. Usually they live in the body without hurting the person or causing any illness, but that is because their numbers are controlled and kept small by the beneficial bacteria also in the body. If a balance upset occurs Candida Albicans Monilia can occur. (Note Monilia is the less used and older term for the Candida and Moniliasis is the older term for Candidiasis.)

Where do Candida Albicans Monilia affect?

Since it is a fungus it prefers areas that are moist and warm, therefore the vagina is a very common place for an infection, other places that are common are the mouth, intestines, throat, anus, penis and in folds of skin. Given the right conditions though Candida Albicans Monilia can occur anywhere on the body, there have been cases of it in the ear, on the face, breasts, nails, fingers and so on.

Factors that cause a Candida Albicans Monilia overgrowth

Low immune system due to a disease or illness like HIV or AIDS

Low immune system due to treatment of an illness such as certain drugs or chemotherapy for cancer.

Uncontrolled diabetes.

Prolonged use of antibiotics.

Living in a humid climate.

Obesity.

Wearing tight clothing.

Wearing synthetic materials.

Poor diet.

Stress.

Working in a damp environment or having parts of the body submerged often in water.

Symptoms if you have an Candida Albicans Monilia infection

Irritation

Itching

Red rash

Yellowing of nails

Degradation of nails

Inflammation

Discharge

Lesions

Treating Candida Albicans Monilia infections

Treating a yeast infection is easier when you catch it straight away. Mild yeast infections can be treated with either over the counter medication or natural remedies. If it is a more severe infection that you have ignored for a while, or if you suffer from recurring yeast infections (4 or more a year) then you need to see a doctor. They will do a proper diagnosis to make sure your problem is from Candida Albicans Monilia and not from a different strain or even a bacterial infection or an STD. If it is definitely a yeast infection, they can prescribe stronger anti-fungal medication. Make sure you finish the course of treatment as the instructions tell you rather than stopping when the symptoms have gone, to ensure the infection is completely treated and prevent a recurrent outbreak.

Preventing a Candida Albicans Monilia overgrowth

Avoid harsh detergents, perfumed soaps, scented products and douching.

Cut out sugar from your diet, it feeds the Monilia.

Take probiotic supplements to top up your friendly bacteria.

Keep yourself clean and dry.

Wear loose clothing made from breathable materials like cotton.

De-stress, sleep well and get some exercise to keep your immune system strong.

Avoid taking antibiotics if they are not completely necessary.

As you may already know if you have done any research on yeast infections, they are caused by a fungus from the Candida family, usually the Candida Albicans species, though not always. Candida can be found in small quantities on most bodies and if kept at low numbers does no harm. However certain risk factors can create a situation that allows the fungus to overgrow which leads to a yeast infection. A yeast infection can also be called candidiasis stemming from the name Candida.

The most well known form of candidiasis is vaginal and for that reason it is perceived as a female infection though this is not the case. An over growth of fungus can cause several types of infections from just annoying to life threatening. If a person gets a yeast infection on their body surface a doctor may refer to it as superficial candidiasis. If it is on the skin, cutaneous candidiasis and if it is in or on mucous membranes such as intestines, mouth or vagina it is a mucosal candidiasis. This article is focused more on mucosal yeast infections and in particular the two types of mucosal yeast infections there are, white form and red form.

White form of mucosal yeast infections

This is called acute pseudomembranous candidiasis. When talking about the white form of mucosal yeast infections the yeast kills epithelial cells by releasing digestive enzymes and other products as it grows on tissue surfaces. This dead tissue then becomes white in color, comes off in clumps and makes a false membrane. When medical text books talk about mucosal yeast infections they usually refer to this form. Thrush, vaginal yeast infection and throat yeast infection or Candida esophagitis are examples of this form.

Red form of mucosal yeast infections

This form is called Chronic atrophic erythematous candidiasis. The difference between the white form and the red form is that unlike the white form, in the red form the yeast is not actively growing. Rather they are invading underlying tissues, and getting inside epithelial cells. When there the fungus releases chemicals that irritate the tissues and the result of this is burning, swelling and redness. The problem with this form is by entering those cells the yeast cannot be reached with anti-fungal medications and the immune system cannot fight it. Denture stomatitis is one example of this form, where the gums are red and burning in those that where dentures. Vulvodynia in women with burning and red vulvae is another. Unfortunately doctors do not always correctly diagnose the red form of mucosal yeast infections.

From benign to life threatening

Here is a look at the spectrum of yeast infections, their severity and the form they take.

Annoying benign superficial candidiasis – this includes the forms of yeast infections most people are familiar with, oral (Thrush), vaginal, penile, colic, denture stomatitis, esophagitis.

Chronic, immune deficiency – chronic granulomatous candidiasis.

Intermediate, candidemia – yeast is in the bloodstream.

Life threatening, systemic, invasive – yeast is in the organs such as the kidneys, spleen, liver.

Ironically many of the risk factors for developing a yeast infection of any kind, not just mucosal yeast infections are caused by the medical world. Treatments for other illnesses can put you more at risk, antibiotics, steroids and other drugs can do this also. Birth control pills for women can be a factor, and equipment at the hospital itself can be a cause.

Candidiasis or yeast infections are most commonly caused by the overgrowth of a Candida fungus called Candida Albicans but that is not the only species of Candida that can cause yeast infections in humans, whether superficial or systemic. After Candida Albicans the species from the Candida family to be the next main cause is Candida Glabrata. Up to 11% of cases are caused by Candid Glabrata. The symptoms and forms an outbreak of Candida Glabrata candidiasis can take are very similar to those of Candida Albicans. The key difference between these two Candida is the treatment.

Diagnosis of Candida Glabrata Candidiasis

If hospitalized it is likely tests well be done when a yeast infection is suspected including cultures taken and urinalyses, though the latter is less accurate. The problem is it can take days for the cultures to grow, days that a patient may not have if they have weakened immune systems with a systemic infection. This is partly the reason for the high mortality rate amongst patients with systemic Candida Glabrata candidiasis. When the culture is grown it is easy to see the difference between Candida Albicans and Candida Glabrata simply in size. Though otherwise similar in appearance in all other ways, Candida Glabrata are smaller.

Despite the fact that Candida Glabrata is second to Candida Albicans in virulence and incidence, not a lot is known about Candida Glabrata as not many studies have been done. It was once known as Tortulosis Glabrata, and some still refer to it as that. It is not something that a doctor can identify from just a pelvic examination. Though superficial cases of Candida Glabrata are rare, if a woman with vaginal candida glabrata candidiasis went to her doctor for diagnosis and treatment, in all probability it would be presumed to be a Candida Albicans infection unless they take swabs and send them off. It is important to get an accurate diagnosis because of how it affects your treatment options.

Treating Candida Glabrata Candidiasis

The problem patients face if they have Candida Glabrata candidiasis is its resistance to anti-fungal drugs. Unlike Candida Albicans which can be treated with a variety of azole drugs, Candida Glabrata candidiasis will not respond as well to commonly used azole anti-fungal treatments like fluconazole or ketoconazole. Candida Glabrata has a more evolved resistance to such treatment, but it can still be treated with other options such as nystatin and amphotericin B, or caspofungin and flucytosine. However for most doctors amphotericin B is usually a last option as a lot of patients do not respond well to it.

Terconazole a 7 day cream has a 40% cure rate when used for vaginal Candida Glabrata candidiasis. Recurring infections are therefore common amongst those rare cases who have this form of yeast infection. This can lead to chronic infections and a problem with the yeast infection spreading to other areas of the body such as the scalp and skin.

Chronic Candida Glabrata candidiasis can be treated with boric acid as an effective second line treatment option. Boric acid needs to be treated with care as it is toxic. Discuss this with a doctor and your pharmacy. Other than this there is little information on Candida Glabrata candidiasis treatment.

Studies

There have not been many studies on Candida Glabrata and more is needed. However in one small study of 33 women with vaginal Candida Glabrata candidiasis 8 responded well to azole treatment alongside boric acid. Only 2 were completely cured and 3 developed a recurring problem that eventually responded to boric acid and flucytosine.

The most well known as well studied species of Candida is Candida Albicans, and this is also the most pathogenic, meaning the Candida species that causes disease the most. When looking at cases of Candidiasis around half or slightly more have Candida Albicans as the causing fungi. They live on healthy people, regardless of gender or age, preferring areas that are damp and warm such as the vagina, the mouth, the throat, the penis and the intestines.

If the Candidiasis is not caused by Candida Albicans then it is likely to be Candida Glabrata, which is the cause of 15% to 20% of cases. This means when you combine the two species between them they are responsible for 70 to 80% of all invasive candidiasis cases. Invasive meaning internal or systemic, where the body’s organs are infected making it potentially life threatening. Candida Glabrata unlike Candida Albicans has become more of a problem amongst those with weakened immunity because it is resistant to several forms of anti-fungal drug treatments. Candida Glabrata is also part of the normal organisms that live on your body, also liking the areas that are damp and warm. However it can also be found in the environment especially in soil, water, leaves and flowers

Forms Candida Glabrata yeast infections can take

Candidiasis caused by Candida Glabrata may also be referred to as Torulopis still in some medical texts. Places Candida Glabrata yeast infections can occur are much the same as the forms Candida Albicans infections such as:

Skin

Intestines

Respiratory system

Mouth

Vagina

Penis

Throat

As with Candida Albicans infections, Candida Glabrata yeast infections can affect both men and women, even children, all the way up to the elderly.

Symptoms of Candida Glabrata yeast infections

Symptoms depend on where the infection is and how severe it is. They include:

Pneumonia

Vaginal discharge white in color

Asymptomatic

Vaginal itching

Vaginal burning

Septicemia

Sore vagina

Urinary pain

Burning when urinating

Cysitits

Meningitis

Fungemia

Feeling of needing to urinate frequently

Cloudy urine

Fever

Bladder pain

Blood in the urine

Breathlessness

Fatigue

Sweating

Difficulty breathing

Coughing up blood

Headaches

Chills

General pain

Despite the fact that Candida Glabrata is the second cause of yeast infections there has been few studies done on it. In one small study of 33 women with vaginal candida glabrata yeast infections the women were split into three groups, the first exhibited yeast infection symptoms, the second had none and the third had other dermatological disorders. Those that exhibited symptoms were observed to have ones similar to an Albicans infection such as redness of the vagina, a white abnormal vaginal discharge and itching.

Conclusion

Mucosal and systemic Candida Glabrata yeast infections are on the rise mostly due to the increase in patients with compromised immune systems. Most incidences of Candida Glabrata yeast infections take a harmless form, but if systemic this is a serious infection. In fact in those with weakened immune systems systemic Candida Glabrata yeast infections can lead to a death rate as high as 77% because of its resistance to many anti-fungal treatments.

Neonatal Candidiasis refers to candidiasis in babies after birth. A baby may be born with a yeast infection or it can develop within a few weeks of delivery. It usually appears in one of two forms, either as skin and mucous membrane lesions picked up when passing through the birth canal from the mother who had a genital yeast infection, or there may be oral candidiasis, otherwise known as Thrush. Both are easily treated unless the candidiasis becomes invasive.

However invasive candidiasis in neonates brings with it a high death rate, some studies reporting as high as 54% and others slightly lower at 28%. Premature babies and those with a low birth rate are at a much higher risk as they need more invasive diagnostic and therapeutic actions which increases the chances of a Candida infection occurring. Since the immune systems are immature, particularly with regards to premature babies, this means they are unable to fight off infections.

For those babies that do present with neonatal candidiasis, up to three quarters of them end up with two or more organs infected. The blood, heart, kidneys, eyes, bones, meninges can all be infected thought the most common presentations are meningitis, renal candidiasis and unifocal osteomyelitis. The Neonatal Research Network has findings that suggest invasive candidiasis occurs in between 1.6% to 4.5% neonatal care units.

Neonatal Candidiasis risk factors

Antibiotics – Length of treatment is a factor as well as type used.

Low birth weight – Out of those babies affected 90% have a birth rate less than 1500 grams (very low).

Congenital malformations including the heart – resulting in longer hospitalization.

Endotracheal tunes and tracheostomies – due to breathing difficulties.

Gastrointestinal tract diseases – which can lead to surgery.

Other factors include catheters and intravenous drips.

Neonatal Candidiasis and Candida species

Out of 4 candida species, albicans, tropicalis, glabrata and parapsilosois, the main one responsible for most neonatal candidiasis cases was Candida albicans. However candida parapsilosis cases are on the rise. Neonatal Candidiasis can sometimes be mistaken for bacterial sepsis with symptoms including:

abdominal distension

hypo-tension

unstable temperature

apnea

decline in respiration (this and apnea are the most common signs, presenting in nearly three quarters of cases).

intolerance to carbohydrates

stools positive for guaiac

What Neonatal Candidiasis can do to parts of the body

1. Central Nervous System

Candida meningitis is one of the most common occurrences and if on a rare occasion the baby does survive there can be brain damage.

2. Mucous membranes and skin

Thrush, diaper rash, skin rash are all possible though not certain. It is possible for invasive neonatal candidiasis to be present yet there be no common rash.

3. Heart

Candidal endocarditis which presents heart murmurs, arthritis, skin abscesses and can even lead to heart failure.

4. Bones and Joints

Arthritis, bone erosion, fusiform swelling.

5. Kidneys

Candidal urinary tract infection, renal candidiasis.

6. Eyes

Candida endophthalmitis.

Treatment of invasive neonatal candidiasis

The most common treatment for invasive neonatal candidiasis is a combination of anti-fungal drugs amphotericin B and 5-fluorocytosine. Side effects are minimal in babies that survive.

A yeast infection is something nobody wants to deal with, but it is a very common issue for women especially, though men can also get them. They are the result of a microscopic fungus that lives on many of us naturally, growing too much. At small numbers it is harmless and is kept low by our immune systems and good bacteria. Over a certain amount though can lead to a yeast infection which is recognizable by the symptoms it presents.

There are several symptoms that are common if you have a yeast infection, itching, redness, swelling, burning when urinating or when having sex and a discharge. A yeast infection discharge is usually the consistency of cottage cheese, and can vary in color from white to yellowish. It should not have an unpleasant odor though, if you have what you believe to be a yeast infection discharge and it has a nasty odor, this is more likely a symptom of a different vaginal infection and you need to visit a doctor to get a proper diagnosis, so that you can ensure you are getting the right treatment.

A yeast infection discharge does not happen in every case, but when it does happen is something that is embarrassing and frustrating to have to deal with. Change underwear frequently and make sure you are wearing loose clothing and that your underwear is made from cotton or another such breathable material. The fungus that causes a yeast infection, Candida Albicans is like all other fungi, it does best in places that are dark, warm and damp. (That is why the genital area and in particular the vagina is so prone to them). By wearing breathable materials the air circulates and keeps the region dry and less hospitable. That way you can prevent symptoms like the yeast infection discharge.

There are treatments available from your doctor or from a pharmacy that will soothe the symptoms and stop the yeast infection discharge. They are anti-fungal in nature and run from 1 day to 7 day courses. Alternatively there are natural remedies you can try such as raw garlic, tea tree oil or even applying plain live yogurt to the area. Different people have different results with the various options available so you will have to do some trial and error before you find what clears up your yeast infection discharge.

A final point to stress is though more common in women men can get a genital yeast infection as well, and experience similar symptoms. Therefore it is possible for a man to find he has a yeast infection discharge, which is much the same as a woman’s in consistency, and remember no smell, or perhaps a yeasty smell, but certainly nothing nasty. It comes from the glans or the head of the penis, and if a man has a yeast infection discharge he needs to get treated as soon as possible.

It can be an awful experience getting a oral yeast infection which is more commonly called Thrush. If you have not ever had it you are lucky and lets hope you stay lucky. For those not so fortunate here are some thrush candida treatments you can do that will help get rid of the infection, and prevent it from ever returning again.

You can tell if you have Thrush or not by the very obvious symptoms of white patches inside the mouth and on the tongue. If you scrape at those patches there are red sores beneath that are very painful. There is often a problem too with bad breath and eating or drinking. The body contains the fungus that causes a yeast infection all the time, but it is controlled in growth and numbers by the immune system and good bacteria. If an imbalance occurs or something acts as a trigger, an overgrowth can happen which leads to an infection. The common places that it can happen on the body are the moist and warm places, so genitals, mouth, anus, under skin folds, under the arms.

As well as getting a prescription from the doctor, there are over the counter candida treatments you can try and there are natural remedies also. A poor diet is one of the large triggers for a oral yeast infection so this is something you can work on yourself in order to prevent it happening. Eating a lot of fresh produce, fruits and vegetables is important for a healthy and strong immune system. Eating sugary foods can help the yeast infection spread because it feeds on sugar. If you do not take care you could develop a systemic yeast infection.

Also try to eat more fiber and less carbohydrates, and less yeasty foods such as beer, bread, cereals and so on. These too feed the yeast. Drink plenty of water to flush the toxins and get enough sleep.

Living on our bodies are many micro-organisms including a fungus called candida albicans and good bacteria. The good bacteria keep the growth of the fungus under control. A yeast infection happens when the natural balance of the body is disrupted and those good bacteria are killed, allowing the yeast to overgrow.

It can affect anywhere on the body but is more common in the mouth, genitalia and intestinal tract. Symptoms vary but include:

•	Lack of energy.
•	Swelling and rashes.
•	An unusual white genital discharge.
•	Fingernail and toenail infections.
•	Itching that is intense and burning.
•	Migraines, fogginess and mood swings.
•	Joint pains and muscle aches.
•	Digestive discomfort and gas.

The pH of your body can be affected which leads to it being more acidic, the ideal environment for the yeast to grow in. Quite often there is not just one yeast infection cause, but several. It is not always easy to diagnose as some symptoms are the same for several possible ailments. Many people rely on over the counter treatments to give relief from those symptoms, but they do not treat the root yeast infection cause. This means people can develop a recurring problem. There are some lifestyle changes we can make to help prevent an infection:

•	Chemicals – Some vaginal sprays and deodrants cause an allergic reaction. Though anti perspirants may kill some of the odor causing bacteria, some of those bacteria are the good ones that control the fungus’ growth.
•	Diet – Eating too much sugar can be a large causing factor as yeast feeds on sugar. Junk food is also bad as it produces the acidic environment the yeast likes. Fruit and vegetables are better because they create a more alkaline environment.
•	Antibiotics – kill good bacteria as well as the bad.
•	Clothing – should be breathable materials like cotton and should not be too tight.
•	Weak Immune system – caused by lack of sleep, stress etc. This combined with one other factor can trigger an outbreak.
•	Warm and damp environment – Make sure you wash after exercise and get out of wet clothing.
•	Condoms – Latex can cause an allergic reaction. Also some have a spermicide or lubricant added that contain Nonoxymol 9 which can trigger an attack.
•	Infected partner – Since it can be passed on during sex, if one partner is infected both need to be treated.

Any woman can suffer from a yeast infection, it makes no difference her age, race or culture. There is already some yeast in the vagina, but it is at low amounts that is safe. Under certain circumstance the yeast can multiply though, leading to a vaginal yeast infection. Sometimes it is an existing medical problem that triggers an overgrowth, some women are more predisposed to them, weakened immune systems, birth control pills, diabetics and even pregnancy can all put you more at risk.

There are some hygiene products that should be avoided too as they can trigger an infection. Douching for example can upset the balance and trigger the vaginal yeast infection to multiply as can perfumed sprays or other scented products used in that area. Though it can be passed on during sex it is not classed as an STD. A candidiasis vaginal infection is relatively easy to cure though if caught early enough.

Symptoms to watch for that indicate there has been a yeast overgrowth are similar to other conditions, so even though it is important to be aware of what these signs are, it is also important to understand you still need to visit a doctor should you observe any of them. They include pain (when urinating and during sex), itching, burning and a candidiasis vaginal discharge that is white and has the consistency of cottage cheese. There is a home test that can be carried out to see whether it is a yeast infection that is bothering you. Have a glass of water next to your bed when you go to sleep and as soon as you wake up the next morning spit a few times into the water. After a while if the spit has cleared into the water you do not have a yeast infection. If it sinks to the bottom or the spit is stringy in the water and cloudy, it is possible that you have a yeast infection. The longer it takes for the spit to string, cloud or sink the more mild the infection is.

After you have a confirmed diagnosis you can begin treatment. A doctor can hep you find the right one for you. Remember to complete the full treatment, do not stop just because the symptoms have gone. For recurring cases you may need to look at your diet and other treatment options in order to become yeast free.

Most doctors can diagnose a yeast infection in otherwise healthy patients, without performing a yeast infection test, though if it is a first infection a test may be performed for accuracy. After a course of treatment is tried, if the infection does not go away then tests may be done.

The yeast infection is the result of an overgrowth of the fungus Candida albicans and is very common in the vagina. But there are other vaginal infections that have similar symptoms and so usually vaginal secretions can be examined under a microscope to be certain. In most cases a yeast infection is not a serious condition, as long as it is diagnosed and treated properly.

The definitive way to diagnose a vaginal yeast infection is by undergoing a full gynecological exam and lab tests being conducted. An instrument called a speculum is used to hold the vagina open and it can be a little uncomfortable due to the pressure. A swab is then taken of the vaginal discharge and cultures may be taken to check for other diseases. The yeast infection test involves the swab being mixed with a drop of potassium hydroxide. If a certain branching pattern can be seen under the microscope it indicates the presence of yeast.

The exam may then lead to the doctor inserting two fingers into the vagina to check for tenderness or problems with the ovaries, uterus or surrounding areas. Blood and urine samples may also be taken. An accurate diagnosis can be made difficult if a woman has sex or douches one to two days before an exam, so avoid both.

When testing toddlers their skin is scraped and then looked at under a microscope. Toddlers typically get yeast infections because they thrive in areas such as a warm, wet diaper. If it seems a toddler has a diaper rash but it has not cleared up after a couple of days then visit your child’s doctor to get it examined. Untreated yeast infections in toddlers can cause great discomfort, so if you want to help your kid you’ll need an accurate diagnosis.

Symptoms vary from one person to another so the best way to be sure you have a yeast infection is to have a doctor diagnose it. Half of people who self diagnose themselves with a yeast infection actually have something else, so whatever symptoms you are exhibiting, get some professional advice.
