Background
The rounded head of the penis is also known as the glans, and Balanitis is an inflammation of that area. If both the glans and the foreskin are inflamed this is called Balanoposthitis. Although Balanitis can be used in terms of an inflammation of the clitoris this article is focused on just the male side infection. It can happen in men at any age though it is more common in boys who are under 4 years old, or in uncircumcised men. 1 out of 25 boys get it and 1 out of 30 uncircumcised men get it making it fairly common. In circumcised men it is very rare.
Symptoms and signs
· An odor that is unpleasant
· Red glans
· Sore glans
· Glans is irritated
· Swollen glans
· The foreskin cannot pull back and is tight
· The glans area is itchy
· A discharge, thick and lumpy beneath the foreskin
· Any soreness, discharge or irritation happens two or three days after sex
· Painful to urinate
As you can see some symptoms and signs are similar to STIs.
Causes
· Skin conditions such as Eczema, Dermatitis, Lichen planus and Psoriasis can lead to Balanitis
· A form of irritation may cause it such as chemicals in condoms, lubricants and spermicides, detergents, fabric conditioners, perfumed soaps and shower gels.
· There are also two types of infections that can cause Balanitis, Candida, a fungus, and bacterial.
· Diabetic men are more likely to get it, particularly if they are not controlling their blood sugar levels.
· Phimosis, which is where the foreskin is too tight can cause it as it may get dirty under there where the man cannot pull it back properly and clean.
· Having unprotected sex with a woman that has a vaginal yeast infection can increase a man’s chances to get Balanitis. STDs like herpes, syphilis and chlamydia can also cause it.
Diagnosis
A doctor will observe the inflammation and redness of the glans. He or she will then discuss possible irritant and good hygiene practice in case those are the causes. They may check for dermatosis and if the condition dies not respond to cleanliness, avoiding irritants and a yeast treatment, they may then do some tests such as doing a swab, taking a urine sample, blood sample, and in very rare cases a biopsy.
Treatment
The cause needs to be identified before a treatment can really be decided upon. If it is an allergic reaction to something a mild steroid cream may be prescribed as well as an anti-fungal cream. If it is bacterial, an antibiotic may be prescribed. If it is Candida an anti-fungal cream like clotrimazole or miconazole may be prescribed and the man’s sexual partner also needs anti-fungal treatment with abstinence from sex while taking treatment. If due to a tight foreskin, circumcision may be the answer and if none of these apply the doctor may refer the patient to a skin specialists, dermatologist.
Prevention
· Keep the glans clean and dry
· Wash daily
· Pull back the foreskin to clean gently in warm water using a gentle soap or none at all.
· Make sure the glans is totally dry before replacing the foreskin.
· If you tend to develop it after sex, was the penis after the act.
· Avoid irritants by using condoms for sensitive skin, using non-biological washing detergent.
· Wash your hands before and after you urinate.
